Your New Patient Packet: Please drop or mail your paperwork and records release back to our office at least 2 week prior to your appointment. We also politely request that you fill the paperwork out completely, including the Records Request, and submit the form to your previous physician. They can then fax your cardiac records to us so that we can get to know your specific needs. To ensure we give all of our patients the highest standard of care, we will have to reschedule your appointment if we are unable to access your paperwork 2 week prior to your appointment.
