We are honored that you chose Dr. Alvarez as your healthcare provider. Our goal is to provide you with great care and service.

Your New Patient Packet: Drop or Mail the paperwork and records release back to our office. We require this information at least two weeks prior to your appointment. Please fill the paperwork out completely including the Records Request and submit the completed form to your previous physician that would have your cardiac records so they can fax these to us. If we do not have these records two weeks prior to your appointment your appointment will be rescheduled. 

Items to Bring to Every Appointment:

Arrival Time: Please make every effort to arrive 30 minutes prior to your appointment. This will give you ample time to complete the registration process, and will allow you to update your demographic, insurance, and health information if necessary.

Late Arrival Policy: If you arrive more than fifteen (15) minutes late, you will be asked to reschedule your appointment(s). We ask that if you are running late, please call and let us know. This will allow us to continue to provide care to the patients that have arrived on time.

The Patient Portal and Healow, our mobile app, enable our patients to communicate with our doctors, nurses, and staff members easily, safely, and securely via the Internet.

Our practice offers the opportunity to use online tools to track your healthcare through our office. The Patient Portal, and our mobile app Healow, enables all patients to communicate with doctors, nurses, and staff members easily, safely, and securely with the click of a button.

By using the Patient Portal, you no longer have to call the office, leave a message, and wait for a response to get the results of your lab work; those results will be available to you through the Portal. You can also send a message to the office through the Portal and expect a prompt reply.
